Clinical Significance
Testosterone functions in the development of male external genitalia, stimulation of spermatogenesis and anabolic activity in a variety of tissues and behavioral and psychological effects. Low testosterone levels in normal females is not well defined. In both sexes, testosterone contributes to the development of secondary sexual hair, acne, and scalp hair patterns.
Excessive production of testosterone, and therefore, high levels in the serum, induces premature puberty in boys and masculinization in females. Decreased excretion in the male is termed hypogonadism and can be due to primary testicular failure, orchidectomy, Klinefelter’s syndrome, hypopituitarism, hyperprolactinuria, or estrogen therapy.
In women, increased testosterone production can result in hirsutim (abnormal increase in body hair), acne, virilization, menstrual irregularities, amenorrhea, and/or infertility. Evaluation of the testosterone helps determine these medical conditions.
Evaluation of the total testosterone is useful in males for erectile dysfunction, osteoporosis, infertility, delayed or precocious puberty and for monitoring replacement therapy. It is useful infants with ambiguous genitalia and/or virilizing syndromes.
